Notice of Injury Form

This is the Notice of Injury form that would need to be completed and sent to the National office in the event of an injury. The completed form can be faxed to 719-598-9440, e-mailed or mailed to USA Wrestling. The area asking for a signature of policyholder, title of policyholder and date will be completed by USA Wrestling.
